In the previous chapters, we discussed the effects of different types of physical activity and exercise training on prevention and treatment of Alzheimer’s’ disease. We will now discuss the effects of cognitive training on improving the general and specific cognitive abilities of Alzheimer’s disease patients by initially focusing on the studies using cognitive training alone as a therapeutic intervention, and subsequently, contrasting the efficacy of this intervention against the combined physical activity and cognitive training protocol. Studies use three main cognitive-based approaches including cognitive stimulation, training, and rehabilitation. However, in some works, researchers tend to use these terms interchangeably . To clarify this confusion in terminology, we will provide a precise definition for each term. According to Clare et al, Cognitive training consists of a series of repeated standardized tasks aimed to trigger a specific cognitive function such as memory and attention. Cognitive rehabilitation, on the other hand, has a patient-centered approach and solely focuses on cognitive weaknesses of the subjects and attempts to improve their ability in performing daily life activities. Finally, cognitive stimulation is the least specific training and consists of a wide variety of social activities targeting improvements in global cognitive performance (i.e., thinking, concentration and memory) as well as social interactions . Background and the Problem Analysis PeopleSharz, an Internet startup company founded in the year of 2012 and the founders of the company are Peter Tweet and Mark Bukerzerg. On 21st April, 2016 the website of the organization was hacked by the hackers and in a teleconference the shared data by the hackers are analyzed and found to be real. From the teleconference the organization came to know that the shared data are similar to the confidential data stored in the database of the organization. The hackers were successful to steal the data from the storage (Rodrigues et al. 2013). Due to this reason the security statement established by PeopleSharz is facing several questionnaires. The main reason behind the hacking was lack of data security or improper usage of encryption keys. The data stored in the database of PeopleSharz used public symmetric key for data encryption but the key was shared key and thus it become easier for the hackers to use the shared key (Fernandes et al. 2014). In order to decrypt the data hackers easily used the shared key. Improper data encryption was the major reason of data hacking. Cryptography is referred to as one of the major area of concern for every organization to maintain the security of the data. Nowadays, the networks used by different organizations are taking global form and information stored in the database is becoming digital (Mahajan and Sachdeva 2013). As information play the vital role, thus open communication channels and stealing of sensitive information are becoming the main target for the hackers. The modern set of cryptography provides robust technology set. However, many benefits and challenges are identified related to encryption (Deligiannidis, et al. 2013). It provides four basic services regarding information security. Encryption technique was used to secure the information and the communication from the unauthenticated users. Authentication is another helpful tool of encryption process which provides authentication such as MAC and digital signature. Digital signature helps to prevent spoofing, snooping and DOS attacks. In addition to this, PeopleSharz encrypted its network traffic to avoid the threat vectors. Introduction of digital signature, and cloud based platform SaaS, PaaS, IaaS to enhance the level of data confidentiality (Li et al. 2013). The website is required to be developed with the help of SSL/TLS. Different operational procedures are adopted to develop the data security of the system. However, PeopleSharz is a startup internet based company, thus the cost estimated or the budget of development was not enough to implement cloud based solution and digital signature process to keep the security of the data. Lack of usage of data security package is one of the major reasons behind the data hacking. The administration or top level management team of PeopleSharz was not using proper data encryption technologies. Virus attacks or malicious attacks on the system administration helped the hackers to steal confidential data from the data storage (Fernandes et al. 2014). The database management system used by the organization must have been used encryption key. By using this technology only the authenticated users access the data stored in the storage. Analysis of the Threats During threat analysis of PeopleSharz different threats are identified and these are as follows: Malware: This is the acronym of malicious software. Malware can either be software or a virus or a worm as well. The hackers generate programming codes and transmit it through website or network to the users device (Parashar and Arora 2013). If the user access those websites or network then the virus carried out by the site will attack the stored data. Then in a rather unforeseen reversal of fortune in the seventh year of Hijrah, the Prophet led a victorious campaign against Kheybar, the hornets nest of the idolatrous Arabs and stronghold of Jewish tribes in North Africa. The Prophet and his army overcame his persecutors, and by the ninth year of the Hijrah, all Arab tribes from all part of Arabia swore allegiance to Muhammad making him the emperor of Arabia. And for their infidelity, the Prophet expelled the Jews from Arabia (xxvi). The seed of a prolonged Jewish and Muslim hostility was sown. There are no absolute rights or wrongs to this case, which is based on a synthesis of this clinical scenario. The aim of this essay is to explore the ethical and cultural issues in nursing practice. Because of these problems, there are more than one solution in clinical settings to preserve human lives free form blood transfusions without violating their beliefs. Ethics is a branch of philosophy concerned with the study of rational processes for decision-making. When the parents’ decision is not the best interest for the child, the paediatric nurse should advocate for him in order to protect his rights to receive treatment according to the Code of Ethics for Nurses in Australia (ANMC 2006). The first step for the nurse in resolving the ethical dilemma like this is to provide information to the parents honestly about the child’s diagnosis, treatments, outcomes and risks. This enable the parents to make free and informed decision (ANMC 2006; Janine William 2010). During the discussion between the health practitioners and the parents, fully explained situation may help the family realize that the decision may not the best choice for the child. Next, health practitioners should strive to remain truly objective and avoid all personal, racial, cultural, religious or other bias when counselling or caring the child and the family. The parents’ autonomy should also be respected during the practice (ANMC 2008b). Thirdly, nurses should respect of the parents’ competence. Competence implies the parents level of understanding that allows them to weigh up the ethical issues posed by a clinical situation, assimilate these and reach a rational decision. Culturally effective paediatric health care can be defined as the delivery of care within the context of appropriate physician knowledge, understanding, and appreciation of all cultural distinctions leading to optimal health outcomes. However, there sometimes has the conflict between the parental authority and the child’s best interests. On the one hand, Australia Nursing and Midwifery Council (2008a) states that nurses should respect the culture, values, beliefs, personal wishes, and decisions of the patients and their families. Moreover, a basic principle in law is that a minor cannot consent to medical treatment until he or she was of or above the age of 14 years old in New South Wales, 16 years old in South Australia or 18 years old in other states. As a result, the parents, or the legally pointed guardians of the child have the rights to make the decision for treatment (Janine William 2010). On the other hand, the patient must always come first, before any vested interes t of any third party including health practitioners as well as the parents, guardians, extended family and society. Brody and Aronson (cited in Tabak Zvi 2008) argue that the patient has the fundamental right to quality medical care and the best treatment. Zohar and Langham (cited in Tabak Zvi 2008) claim that parental consent to medical treatment may negate patient autonomy. In a conflict situation as described here, when informed consent is not forthcoming, the nurse’s important role in working is to seek an alternative decision (Tabak Zvi 2008). Where a parent or guardian refuses consent to use blood products in the emergency treatment of a child (less than 16 years of age), the local, state, territory or national legislation or guidelines in regarding consent for a medical procedure must apply (Australian and New Zealand Society of Blood Transfusion Ltd 2011). Slonim et al. (2008) states that the administration of blood products to children is a common practice in acad emic childrens hospitals; Complications associated with these transfused products are rare. When blood transfusions are deemed necessary for an immature minor. Jehovah’s Witnesses believe that receiving blood from other people will damage their relationship with God. However, it is true that acute blood loss has been associated with increased mortality for decades; a blood transfusion is the best way of replacing the blood quickly (BloodSafe 2008). Due to this, several methods have been used to protect their autonomy in emergent situations where blood or blood product may be used. Office of the Public Advocate (2010) introduces the guideline of Jehovah’s Witnesses and Blood Transfusions to assist physicians and other health workers to deal with this kind of dilemma based on Guardianship and Administration Act 1986, Medical Treatment Act 1988 and Human Tissue Act 1982. In Victoria, most Jehovah’s Witnesses have a â€Å"No Blood Card†. It indicates that the person who hold this card will not receive blood or blood products in no circumstances (OPA 2010). Furthermore, there are blood transfusion alternatives, and pr actices that use no blood for elective or non-emergency surgery or as a result of traumatic injuries to address the medical needs of patients who did not wish to receive blood products or blood transfusions during medical care. The 2009 Association of Anaesthetists of Great Britain and Ireland guidelines recommend the method of cell salvage in cases where patients have refused to use allogeneic blood and blood products (cited in Ashworth Roscoe 2010). Goldberg and Drummond (2008) states administration of recombinant activated factor VII (rFVIIa) is effective solution to treat Jehovah’s Witness patients with life-threatening bleeding associated with haemophilia or trauma. Schmitt el at. (2008) recommend the use of autologous peripheral blood stem cell transplantation for high-dose chemotherapy without support of allogeneic blood products. In conclusion, clinical problems with significant ethical and cultural implications pose an ever increasing dilemma in everyday medical practice in the 21st century and rarely present a simple solution. The paediatric nurses’ role is to advocate the child’s best interest when the parents’ decision is contradictory. Individuals’ religions and cultural beliefs should be respect. In clinical settings, there are increasing options other than blood product transfusions for those who refuse blood transfusions. One advantage is that companies are able to greatly reduce the amount of paper that they use. Not only does this help the environment, it helps cut costs within the organization. Companies are also able to improve service through implementing the paperless office. This is because communication is immediate and does not get lost in a pile of papers on someone's desk. A paperless office can also save the company money. This can be seen in the example of Washington Mutual Savings Bank of Seattle. The bank automated more than one-hundred different forms and estimates that they are saving upwards of one million per year.   Ã‚  Ã‚  Ã‚  Ã‚  One disadvantage to having a paperless office is the issue security. How does a company make sure that only the eyes the document is intended for, are the only eyes that see it? Also how does a company know an electronic communication is authentic? Another issue is privacy. How does a company make sure that when an electronic communication is sent only the person it is intended for will read it? How does a company make sure private information does not make the evening news? 2. Are certain types of information more readily amenable to digital processing in a paperless office than others? Ebenezer Scrooge is a tightfisted miser who has only one purpose in life, to extort as much money and profit he can from anything and everything. As with all things, too much of one thing is bad for you; Scrooge’s miserly ways are catching up with him. His cheap ways have not brought him any friends, quite the contrary; they have brought him derision and scorn. He was thought of as â€Å"a tight-fisted hand at the grindstone! † A squeezing, wrenching, grasping, scraping, clutching, selfish, covetous old sinner! As we can see, he wasn’t a very pleasant person, but that is to be expected of people who work around money all their lives. Money became more than a possession to Scrooge, all his coins were his little children. He kept them safe in their strongboxes. To give away but one petty coin, would have been asking Scrooge to give away part of his soul. He was greedy and crooked to the bone. He was a cold heartless man â€Å"No warmth could warm†¦ No wind that blew was bitter than he. † Scrooge hated the idea that on Christmas day his workers were allowed the day off and Scrooge still had to pay their wages. â€Å"A poor excuse for picking a mans pocket every twenty-fifth of December. † Scrooge dislikes people who try and raise money to help the poor, when the charity workers tell Scrooge some poor people would rather die than work in a workhouse Scrooge replies. â€Å"If they would rather die†¦ They had better do it, and decrease the surplus population. † This quote shows that Scrooge doesn’t care about other people apart from himself. He doesn’t care that other people are worse off than he is and he doesn’t care if they die. â€Å"If I could work my will†¦ Every idiot who goes about with merry Christmas should be boiled with his own pudding. † This is a quote from Scrooge before the ghosts visit him. It shows that Scrooge doesn’t like Christmas and doesn’t like other people enjoying themselves at Christmas. Scrooge wasn’t just miserable and bad tempered at Christmas; he was like that all year round. These famous lines were uttered by Scrooge on Christmas Eve, 7 years to the day, of his old partner, Jacob Marley’s death. Then as Scrooge was about to enter his marvelously slum-like mansion, he looked at his doorknocker, and nearly fainted dead away with fright. â€Å"Scrooge†¦ saw in the knocker†¦ not a knocker, but Marley’s face. † Bad omens breed ill times to come, and this was most definitely a bad omen. â€Å"As Scrooge looked fixedly at this phenomenon, it was a knocker again. † Scrooge just thought it was the dark playing with his mind. â€Å"Darkness was cheap and Scrooge liked it. † When Scrooge sat down by the small fire the cellar floor blew open with a booming sound, still Scrooge wouldn’t believe anything strange was happening. â€Å"Its still humbug†¦ I wont believe it. † When the ghost of Marley appears before Scrooge he still doesn’t accept what he’s seeing. The ghost asks Scrooge â€Å"Why do you doubt your senses? † Scrooge replies, â€Å"Because a little thing effects them†¦ there’s more of gravy than of grave about you. † Marleys ghost explains to Scrooge that he is forced to travel around the world; he sees people suffering and cant do anything to help them. Marley’s ghost explains to Scrooge that he is forced to do this in death because of the way he was in life. Marley tells Scrooge that his punishment after life will be even greater because Scrooge was just as bad as Marley and has had an extra seven years of life to do bad things. Marley told Scrooge that unless he changes his ways he will be punished. He tells Scrooge that three spirits will visit him. â€Å"Expect the first tomorrow when the bell tolls one. † The ghost of Christmas past was a strange figure. Its hair hung about its neck and was white as if with age. Its arms were very long and muscular. It had a lustrous belt around its waist and was holding a branch of fresh green holy in its hand. â€Å"You are one of those who’s passion made this cap, and force me through whole trains of years. † The ghost said to Scrooge. Meaning because of people like Scrooge the ghost has to visit them. The ghost of Christmas past took Scrooge to the place where he grew up. They saw travelers; Scrooge knew and named every one of them. â€Å"Why did his cold eye glisten†¦ Why did his heart leap as they went past. † The ghost then took Scrooge to his old school; he was the only child left there. All the other children had gone back home to their families for Christmas. The ghost then took Scrooge to a house were a young Scrooge was reading a book by a weak fire. Some actors visited young Scrooge and performed a play for him. The difference between a vision and a goal, one is abstract and one is concrete. Visions are essentially abstract, while setting a goal is more tangible. For an example of a vision, one would say they would like to plant a large garden that will produce a vast harvest In the spring; It will contain green beans, tomatoes and corn. We have dreams in life and these dreams are the things that we aspire to have, the status that we want to establish, or the kind of life that we long to have in the future. Thus, at times we are daydreaming that we are there, about to fulfill our dreams. In Mark twain’s The Mysterious Stranger Satan was mentioned as ghost standing at the behind the food processor and admiring a person’s lawn-mowing skills. It was not clear whether it was a dream but dream sometimes appears as one’s night mare. In the ensuing conversation between Satan and the person, Satan says â€Å"dream other dreams, and better† (120). Here, it was not clear whether Satan was advising the person to have a better ambition in life or he was just inciting a person to think of something beyond what he can do in order to make sin. But what ever is Satan’s intension, there is a truth in his statement. One should really have dreams life, a better dream. Dreams as some psychologist say are simply product of our imagination. They say, when we are asleep, our sub-conscious mind becomes active which us to dream dreams. No one likes bad dreams, but everyone enjoys good dreams. In dream, we can do anything that is impossible in real life and when we are awakened we regret that it was only a dream. The truth is we can dream a better dream that is not merely product of our subconscious mind when we are asleep. Our dreams can turn to reality if we could overcome anything that limits us. Our dreams can be fulfilled if we let all our strength, courage, strength, wisdom, and all our best efforts to pull together for a single purpose, achieving our better dreams in life. The scenario led us to think of something dreadful. Theodor said to Satan, â€Å"It was ill time to laugh, for free and scoffing ways†¦brought him under suspicion. † Satan protested in this statement saying, â€Å"There spoke the race† â€Å"always ready it hasn’t got †¦ you have perception of humor, nothing more; a multitude of you possess that. This multitude sees the comic side of a thousand low-grade and trivial things-broad in-congruities, mainly: grotesqueries, absurdities, evokers of the horse-laugh† (109). There is something we can learn from this statement. Many people see only the comic side of the things around them or of the people they met and laugh at them. We tend to be evokers of the horse-laugh rather than objective in our judgment of anything that we see on others. Most of the time, we laugh at people even when it is an ill time to laugh. Perhaps, our problem is that we could hardly see when the right time to laugh on other people is. If I’m shifting my service user from chair to bed so I have to check the brakes of the chair and the bed so he/she should safe from risk. If the floor is wet so I have to put wet sign on the floor so protect me and my co-worker from any danger, harm. E.g. one morning it was giving wash to my service user and I left the shower gel out the cupboard after few seconds I saw my service user trying to open it I took it from her and I locked in the cupboard. Now onward whenever I give wash to my service user I kept things properly. You should give equal right to every service user because they all are equal to us. E.g. I have to encourage my service user if they wants to go to shopping and express their needs and preferences so they feel they are independence .They have right to express to their needs and preferences. I have to promote their health and care by telling them to have proper meal and drinks, encourage my service user to eat whatever they want, according to care plan. If a diabetes service user likes sugary tea I have to explain to her or him that it is not good for your health, it is very risky to your health. When we all are working together so sometime conflicts occur we should resolve between each other so we can work properly. If we can solve it we should go manager and seek for help. I should involve my service user in activities e.g. playing games,cards,music,painting,but while activities I should see the risk,e.g if I’m playing games with my service user I have to stay with them because they can put anything in their mouth or whole painting they drink the liquid. So environment should be hazard free. I have to store equipment e.g. if I’m giving wash to my service user and I left washing liquid outside the cupboard so he/she can drink the liquid so I have to store the liquid when I finish my work. E.g. when I go to work and attend my service user to get out of the bed I should wash my hand before I give wash to my service user and after giving him/her wash I should wash my hands to control the contamination and infection. In my work of place, i kept all hazardous and no-hazardous things in proper place and equipment in proper place and waste should be in clinical waste, like gloves, apron in yellow bag so we should protect me and service user, co-worker from any dander and harm. E.g. if I’m going to get my service user out of the bed so first of all I should check the brakes of the hoist, brakes of the wheelchair, batteries of the hoist, so I protect me and my service user from any harm. In my work of place, e.g one time suddenly fire occur some of the area in the nursing home, so we all come to assembly point we check the fire panel and we discover fire is which Zone so one of them go upstairs to check the Zone but when we discover it was false alarm, Now we all know if in emergency what should we do. E.g. if fire occur -arise the alarm -call 999 -inform staff on duty -never use the lift. Call for help shoutly -never leave the service user alone -make recovery position E.g. if fire happened I should break the glass so everybody knows that it’s a fire alam.call for help shoutly clear the environment, do not use the lift.

The Timeless Truth of Madame Bovary  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚      Written in 1857, Gustave Flaubert's Madame Bovary has become a literary classic. Emma Bovary is a middle class country girl with a taste for rich things; she marries a doctor and has a little girl. Her husband, Charles, adores her and thinks that she can do no wrong. He overlooks the sign of her adultery, telling himself that her unhappiness is caused from her poor health, and forgives her excessive spending. Madame Bovary's excessive desires seem to come from her excessive reading of novels in which life seemed, to her, perfect. She "tried to find out what one meant exactly in life by the words felicity, passion, rapture, that had seemed to her so beautiful in books" (45). Through Emma, Flaubert illustrates that not being satisfied with what one is given in life leads to a sorrow. Soon after Emma marries Charles, she finds that she is not satisfied with her new life, due to Charles' lack of romantics. Emma thinks to herself early on in the marriage, "A man, . . . should he not know everything, excel in the manifolds activities, initiate you into the energies of passion, the refinements of life, all mysteries? But this one [Charles] taught nothing, knew nothing, wished nothing. He thought her [Emma] happy; and she resented this calm, this serene heaviness, the very happiness she gave him" (54). Her need for Charles to be more romantic and his ignorance of her feelings lead her to despise him. After a few years of their marriage, Emma has become so bored with her life that she has made herself sick from want. Her boredom is so great that she wishes she could talk to her servant, "but a sense of shame restrained her" (81). She held herself above everyone, therefore isolat... ...ath does Emma come to realize that the best things in life is family and the happiness that it can provide. The selfishness that had ruled her life was nothing now all the things that were importune to before are now nothing. The things she had bought and the lovers she had been with are not with her now. Only Charles and her little girl, the ones she had tried to flee from are with her now. The simple truth portrayed in Madame Bovary still pertains to the present, selfishness will lead to a life of discontentment. The Flaubert illustration of the unhappiness that thinking only of oneself can bring to others can still be seen in the world today.
